The transfer bag scheme is an initiative concerned with transferring care home residents safely when they need to go into hospital. It makes sure clinicians have relevant information to make decisions about the persons ongoing care based on up to date information and the residents own wishes. The bag facilitates the sharing of information to ensure better care for the resident in the hospital and also helps facilitate the safe transfer of the resident’s belongings in an out of hospital. Key information relating to the residents care is shared by the care home to the hospital, this is a two way process and any changes in a residents care, treatment and/or medication can be shared back to the care home in a timely fashion. This facilitates high quality seamless care to residents who have complex needs.
